Choice Health at Home Acquires 3 Home-Based Care Businesses, Enters New State

Choice Health at Home is continuing its expansion strategy with the acquisition of three home-based care businesses, extending its services across the full in-home care continuum and entering a new state. The Tyler, Texas–based provider announced it has acquired Cy-Fair Health Care, Alliant Home Health, Palliative and Hospice Care, and Senior Nannies Private Care.

The acquisitions strengthen Choice’s presence in existing markets and expand its footprint into Florida. Cy-Fair Health Care, based in Tomball, Texas, expands Choice’s reach across Southeast Texas by offering skilled nursing, wound care, therapy services, and diabetic education. Meanwhile, the acquisition of Alliant broadens Choice’s operations in Colorado, particularly in the Denver, Westminster, and Colorado Springs areas, building on earlier hospice and personal care investments in the region.

Choice also acquired Senior Nannies Private Care, a Florida-based provider offering skilled nursing, personal home care, and assisted living placement services. Company leadership described Florida as a strategic anchor for future growth throughout the Southern U.S., with Senior Nannies playing a key role in integrating personal care and clinical services.

CEO David Jackson said the acquisitions reflect Choice’s vision of delivering coordinated, multi-sector home-based care, noting that combining personal care with skilled clinical services leads to better patient outcomes and improved quality of life.
